Open Vaccination Clinic

Desert Cares Network will be providing a vaccination clinic at the Center. There are some forms to fill out, but it is open to all of our clients, clients spouse/caregiver. Please bring your State issued identification card or driver’s license, and a copy of your health insurance card.

You can pick-up forms a head of time at the Center, or you can request them to be emailed to you. Available in both English and Spanish.
